Can someone PLEASE PLEASE PLEASE post #12's steps?

I cannot find a complete guide to the steps for #12... the neo boards aren't loading for me and the help guides I can get too on neo aren't updated as I'm sure the authors can't get the site to work either.
So... complete step by step guide to #12 to anyone who's gotten it? THANKS!!
